Funeral Benefit
claim


If a valid death claim is admitted, the claim amount will be paid to the following people in the following order:
  • The beneficiaries for proceeds (if any).
  • The policyholder, if different to the life insured.
  • The deceased estate, if the life insured and policyholder is the same person.

Funeral Benefit claim requirements

  • A copy of the death certificate (BI-5).
  • A copy of the deceased’s identity document or birth certificate (if younger than 18 years).
  • A copy of the Notification of Death (DHA-1663) Form obtainable from the doctor who declared the death.
  • The Funeral Claim Form completed and signed by the claimant or duly authorised representative of the claimant.
  • If death is due to unnatural causes, the Unnatural Death Claim Form is also required and must be completed by the Investigating Police Officer.
  • The POPIA Consent Form completed and signed by the claimant or duly authorised representative of the claimant.
  • Proof of banking details, in the name of the claimant (natural person, minor, cessionary, deceased estate, legal entity), not older than three months on a bank letterhead or that has a bank stamp on it.

And the following additional requirements:

If the claimant (beneficiary) is a natural person

  • A copy of the claimant’s identity document
  • Proof of payment for funeral costs - if the claimant is not the policyholder and the policyholder is deceased. (Only for Southern Life (SL) and Sage (GE) policies).

If the claimant (policyholder) is a natural person

  • A copy of the policyholder(s) identity document(s).

If the claimant (beneficiary) is a minor

  • A copy of the legal guardian’s identity document.
  • And the legal guardian is a biological parent, we require a certified copy of an unabridged birth certificate, which reflects both parents’ names.
  • And the legal guardian is not the biological parent, official proof of guardianship as issued by the Master of the High Court or Children's Court and a copy of the birth certificate

If the claimant is the deceased estate

  • A copy of the legally appointed executor’s identity document.
  • A copy of the letter of executorship or Article 18 (3) letter of administration.

If the claimant (beneficiary) is a foreign national

  • A copy of claimant’s passport
  • A copy of the claimant’s naturalisation certificate, if born in South Africa
  • Exchange Control Form MP336 (b) (an application to the South African Reserve Bank for foreign capital allowance) completed in full and signed by the claimant.

  • OR
  • Written confirmation that the claimant never lived in South Africa and was not a South African resident.
  • Declaration by the Claimant Form
  • The name of the bank and branch where the claimant banked in South Africa, before departure.

We may require additional information for medical requirements and legal entities.
