Critical Illness Benefit claim


The critical illness cover benefits provide financial protection against illnesses such as a heart attack, cancer, stroke, Alzheimer's and Parkinson's disease. You are also covered for injuries from accidents such as paraplegia, major burns and brain damage.

Critical Illness Benefit claim requirements

  • The Critical Illness Claim Form (specific to the cardiac and arterial, cancer and nervous systems) completed and signed by the policyholder or duly authorised representative of the policy holder.
  • Or
  • The Critical Illness Claim Form (general - other) completed and signed by the policyholder or duly authorised representative of the policyholder.
  • The POPIA Consent Form completed and signed by the claimant or duly authorised representative of the claimant.
  • A copy of the life insured’s identity document.
  • Proof of banking details, in the name of the claimant (natural person or legal entity), not older than three months on a bank letterhead or that has a bank stamp on it.
  • Copies of all the relevant diagnostic tests or reports.
  • An up-to-date confidential medical report from the treating specialist, which must include the full diagnosis, prognosis, treatment plan and response to treatment. Regrettably, a report from a general practitioner will not be accepted.
  • Curator bonis appointment if the claimant is not able to handle his/her own financial affairs (if applicable).

And the following additional requirements:

If the life insured is a minor

  • And the legal guardian is a biological parent, we require a certified copy of an unabridged birth certificate, which reflects both parents’ names.
  • o And the legal guardian is not the biological parent, official proof of guardianship as issued by the Master of the High Court or Children's Court and a copy of the birth certificate.

If there is a cessionary

  • A copy of the authorised representative’s identity document.
  • A written instruction from the cessionary confirming the bank account to use for the claim payment as well as proof thereof not older than three months on a bank letterhead or that has a bank stamp on it.

If the claimant is a trust

  • A copy of the trustee’s identity document.
  • A copy of the letter of authority issued by the Master of the High Court.
  • A copy of the trust deed and resolution.

If the claimant is a company

  • A copy of the authorised representative’s identity document.
  • A copy of the CK1 documents and resolution

We may require additional information for medical requirements and legal entities.
